How the program works:

There are two core components to our program:

Preventive requirements

Age and gender appropriate screenings such as:

  • Annual preventative exam
  • Colorectal cancer screening
  • Mammogram
  • Flu vaccine
  • Dental cleaning

Chronic care education and counseling

Access to clinical and lifestyle coaching from a nurse and educational materials:

  • Diabetes
  • Heart disease
  • Heart failure
  • Asthma
  • Chronic Obstructive Pulmonary Disorder (COPD)
